The intake form asks for your name, organization, email, phone number, affected systems, timing, authorization confirmation, and a short description of what happened.
Do not submit passwords, MFA codes, recovery codes, financial account credentials, full payment card numbers, Social Security numbers, protected health information, or other highly sensitive regulated data.
Request information is used to review the situation, decide whether the service appears appropriate, contact you, and provide triage if you choose to proceed.
The site uses Cloudflare for hosting, Turnstile protection, and serverless form processing. Form notifications are sent through Mailgun. Google Analytics may be used to understand site traffic and form performance.
The public site does not intentionally store form submissions in a database. Operational notes, emails, invoices, and written summaries may be retained for business records and service continuity.
